Now offering DOT (Department of Transportation) exams. DOT exams are included at no additional fee with any active membership with Meredith Bertrand. Non-members can book DOT physical exams for $100
Important to know *
Once you have enrolled on SigmaMD, you will be able to schedule your DOT physical through the chart app.
You will be sent an invoice for the physical. The invoice must be paid BEFORE the exam. If the invoice is not paid within 24 hours prior to the exam, the appointment will be cancelled.
You can reschedule or cancel your appointment as long as it is at least 72 hours before the exam. If it is less than 72 hours prior to the exam, then the $100 appointment cost will not be refunded. The $25 enrollment fee is non refundabl
Make sure you bring any important information and test results including sleep study results and CPAP compliance reports, required waivers, and specialist notes.
