Comprehensive Primary Care and Chronic Disease Management
Personalized primary care focused on preventive care, early diagnosis, and long-term health. ​Ongoing care for common chronic conditions including high blood pressure, diabetes, high cholesterol, and thyroid disorders. When specialized care is needed, coordinates referrals to specialists and remains involved in care

Acute Illness & Same-Day Care
Evaluation and treatment of common acute illnesses such as respiratory infections, sinus infections, urinary tract infections, minor injuries and basic wound care, gastrointestinal symptoms, and other new health concerns.

Lifestyle & Preventive Health Counseling
Support for improving overall health through nutrition guidance, exercise planning, sleep optimization, and stress management. Medical guidance for weight loss and metabolic health, including lifestyle strategies, nutrition counseling, and weight loss medications when appropriate.

Mental Health Care
Evaluation and treatment of common mood disorders such as anxiety and depression. Ongoing maintenance of ADHD medications for patients with an established diagnosis who are stable on their current treatment plan.

Women’s Health Services
Women’s health care in the primary care setting, including preventive screenings, Pap smears, contraceptive counseling, menstrual concerns, and menopause management. Evaluation and treatment of common gynecologic conditions.

Hormone Replacement Therapy
Initiation, adjustments of, or continuation of HRT for treatment of menopausal/perimenopausal symptoms in women, testosterone replacement therapy in men, and in gender affirming care

Acute and Chronic Pain Management
Treatment of acute pain conditions and non-opioid chronic pain management strategies.

Dermatologic Care
Diagnosis and treatment of common skin conditions including acne, eczema, rashes, fungal infections. Basic procedures including treatment of warts, skin biopsies, draining of small abscesses, and suture removal

In-Home Primary Care
One home visit is done every 6 months with the home-care membership. In-clinic appointments can still be done as often as needed. The home care plan can also be done transitionally. (For example, if someone is being discharged from rehab and has limited mobility to be able to come into the clinic initially).
